Do I have to give a recorded statement to the insurance company if I suffered a catastrophic injury?

You do not have to give a recorded statement to the insurance company generally speaking. The only exception to that would be if your insurance company is involved, if it’s an auto accident. Then you have a duty to cooperate. But generally speaking, I would not give any recorded statement to an insurance company, particularly unless you consult with an attorney to make sure that it’s appropriate given the circumstances.
